The grants enable eligible entities to create, coordinate, implement, and evaluate cross-sector plans to improve the health, well-being, and quality of life of covered individuals.

The grant focuses on the care needs of older individuals, including older individuals with disabilities; their family members; paid and unpaid caregivers; and older relative caregivers. Targeted subgroups are those who have the greatest social or economic need as a result of location, disability status, sexual orientation, gender identity, gender, socioeconomic status, language, immigration status, or other factors.

The Assistant Secretary shall award up to 65 grants over the 5-year period after bill enactment. At least 10 grants will be made to Indian tribes or tribal organizations. The maximum award amount to an eligible entity is $500,000.
