Program Details
Overview: The bill authorizes a three-year grant program for eligible organizations offering home visiting programs for family caregivers. These include local government agencies; health care entities; and nonprofit or community organizations that conduct initial home visits for family caregivers. Services include caregiver assessment; education and training; referrals to appropriate local programs; and referrals for physical and mental health care.
Application
An eligible organization submits an outreach plan that describes how they will identify family caregivers most in need of support, including those at risk of needing institutional care. The application describes how grant funds will be used. Entities must have expertise in: caregiver assessments; education and training; and support for home modifications. They must be able to provide or refer caregivers to programs for physical and mental health care needs and support services such as: transportation; home modification; respite care; adult day services; support groups; and legal assistance. They must coordinate with other state and community-based organizations and engage caregivers, family members, and care recipients.
Priority
Priority is given to eligible organizations with the greatest likelihood of providing suitable family caregiver home visiting services for their community. Organizations that use evidence-based programs and/or provide matching funds are also prioritized.
Technical Assistance Center
The Secretary will establish a technical assistance center, carried out by the National Caregiver Support Collaborative Technical Assistance and Coordinating Center. It provides evidence-based models; training for grantees; and facilitates information exchange among grantees, and other federal programs.
Evaluation
Within two years, the Secretary will evaluate the grant program, measuring: reduced hospital and nursing home use; cost reductions; and improvements in care access care and quality of life.
